If your player is part of either the Minnetonka, Edina, St. Louis Park, Robbinsdale, Hopkins, Wayzata, or Eastern Carver County school districts, your instrument will be available for pickup at House of Note one week before the start of the selected session.

All instrument returns and exchanges must be scheduled by the customer with House of Note at 952-929-0026. Instruments left at school without a scheduled pickup with House of Note are not considered returned.
Digital Signature
I, the undersigned, understand and agree to the below terms and conditions and hereby give permission to House of Note to charge my credit card all rental fees, accessories, insurance and sales tax.
By selecting the "I Agree" checkbox below and printing my name, and date, I agree that the name represents me the signer, and will be the electronic representation of my signature for this HOUSE OF NOTE RENTAL CONTRACT, and legally binding me, the signer, to all terms and conditions of the rental contract.
RENTAL/PURCHASE AGREEMENT TERMS & CONDITIONS:
Insurance: House of Note will cover replacing broken strings, any gluing of openings, bridge and peg adjustment, intonation problems, and adjustments needed for bows and cases. House of Note will also cover theft, loss, and damage. Any claims for stolen instruments must be accompanied by a police report. House of Note will not cover any instrument stolen from or damaged while in a motor vehicle, nor will we cover intentionally damaged instruments. House of Note will not cover theft or damage incurred while traveling by airplane or other public transportation. All repairs must be performed or authorized by House of Note.
Extended Insurance Coverage: The cost of insurance coverage described above is $4.00 per month for Violin/Viola and $6.50 per month for Cello. Coverage becomes void if rental payments become 30 days delinquent. The renter is responsible for maintaining the instrument to a good playing condition until it is returned.
Equity Towards Purchase: IT IS AGREED THAT SHOULD THE CUSTOMER DESIRE TO PURCHASE THIS INSTRUMENT, OR ANY OTHER INSTRUMENT FROM HOUSE OF NOTE, THAT 100% OF THE FIRST 12 MONTHS RENTAL FEES WILL BE DEDUCTED FROM THE PURCHASE PRICE. INSTRUMENTS PURCHASED FROM HOUSE OF NOTE RETAIN AN 80% TRADE IN VALUE. Customer may return the instrument to House of Note at any time. Fees are prorated if returned in the middle of the rental month. Cello rental credit applies towards the purchase of a Cello only.
Past Due Account Policy: The customer will be charged for one month’s rental fees about two weeks before the end of the initial rental period. After that we will charge every other month for two months at a time. If your account becomes 30 days delinquent, a $5.00 late fee will be added per month, per instrument.
House of Note reserves the right to use any remedy deemed necessary (including lawsuit) to take possession of non-returned instruments and to collect all past due rental fees, late fees, legal fees, collection costs and losses due to uninsured damage.
